1. VOID WHERE PROHIBITED BY LAW. ALL FEDERAL, STATE, AND LOCAL LAWS AND REGULATIONS APPLY. Void outside the 50 United States, District of Columbia and where prohibited. This contest is limited to youth and high school football organizations located in select counties in New York, New Jersey, and Connecticut (defined in section 3 below) at the time of entry. Entries on behalf of organizations may only be submitted by residents of select counties of New York, New Jersey and Connecticut who are at least 18 years old at the time of entry. Entry requires internet access and the capacity to upload documents, videos and other materials. By entering and participating in the Contest, Entrant and Finalists (defined below) each agree to be bound by these Official Rules and the decisions of the Sponsor, which are binding and final in all matters relating to this contest. NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING.
  2. PARTICIPATION CONSTITUTES ACCEPTANCE OF RULES AND DECISIONS, CONSENT TO USE AND RELEASE OF CLAIMS. By entering the 2016 CHASE Football Town Contest (the "Contest") sponsored by USA Football (“USA Football” or “Sponsor”) and presented by JPMorgan Chase Bank, N.A., 270 Park Avenue, New York, New York 10017 (“Chase”) and submitting additional materials as a Finalist, each Entrant and Finalist warrants that he or she has read and agrees to abide by all contest rules ("Official Rules") and that he or she is not prohibited from participating in the Contest. Participation in this Contest constitutes Entrant’s and Finalist’s full and unconditional acceptance of these Official Rules and the decisions of USA Football, its subsidiaries, affiliates, employees and agents, which are final, binding and non-negotiable in all respects.

Each Contest Entrant, Nominee, and Finalist agrees to release, discharge, indemnify and hold harmless Sponsor, Chase, the New York Football Giants, the National Football League and each of their subsidiaries, affiliates, agents and each of their officers, directors, employees, shareholders, representatives and advisors (the “Released Parties”) from any liability arising from or related to the Contest and/or the receipt or use of any prizes awarded in connection with the Contest, including without limitation, personal injury, death, and property damage, and claims based on publicity rights, defamation, or invasion of privacy. Further, Entrant fully and unconditionally releases all claims for consideration, copyright infringement and all other claims of any nature relating to the use of Entrant's entry, name or likeness.

  1. The Contest is a regional contest open to youth and high school football organizations that are residents of the following counties in New York, New Jersey, and Connecticut:

                  New York counties: Albany, Broome, Cayuga, Chemung, Chenango, Clinton, Columbia, Cortland, Delaware, Dutchess, Essex, Franklin, Fulton, Greene, Hamilton, Herkimer, Jefferson, Lewis, Madison, Montgomery, Nassau, New York City, Oneida, Onondaga, Orange, Oswego, Otsego, Putnam, Rensselaer, Rockland, Saratoga, Schenectady, Schoharie, Schuyler, Seneca, St. Lawrence, Suffolk, Sullivan, Tioga, Tompkins, Ulster, Warren, Washington, Westchester.

                  New Jersey counties: Bergen, Essex, Hudson, Hunterdon, Mercer, Middlesex, Monmouth, Morris, Ocean, Passaic, Somerset, Sussex, Union, Warren.

                  Connecticut counties: Fairfield, Hartford, Litchfield, Middlesex, New Haven.

Entries may only be made by residents of these New York, New Jersey and Connecticut counties who are aged 18 or above at the time of entry (“Entrant”).

Additionally, each nominated youth football organization (each a “Nominee”) must be affiliated with a USA Football Heads Up Football (“HUF”) member league. Membership in HUF not required for nominated high school football teams (each a “Nominee”). Employees, officers, representatives of the Sponsor and Chase, their affiliates and each of their immediate family members (regardless of where they reside) or household members, whether or not related, are not eligible to be an Entrant and enter a Nominee or serve on the board of directors for any nominated organization. “Immediate family members” shall mean parents, step-parents, children, step-children, siblings, step-siblings, or spouses. “Household members” shall mean people who share the same residence at least three months a year. Selection as a Finalist, as defined below, or winning a prize is contingent upon fulfilling all requirements set forth herein. VOID OUTSIDE OF THE UNITED STATES AND WHERE PROHIBITED.

  1. CANCELLATION, TERMINATION OR SUSPENSION OF CONTEST. Sponsor reserves the right at their sole discretion to cancel, terminate, suspend or amend this Contest at any time for any reason without prior notice. Sponsor further reserves the right to amend these Official Rules at any time without prior notice, in their sole discretion. Sponsor reserves the right to disqualify any Entrant, Nominee, or Finalist for any reason, including, but not limited to, violation of these Official Rules, suspected voting fraud on behalf of an Entrant, or infringement of the rights of a third party. Sponsor is not responsible for any printing or typographical error in any material associated with the Contest.


  1. CONTEST PERIOD. The Contest will run from 5:00 p.m., E.S.T. on Thursday, September 22, 2016 until 12:00 p.m., E.S.T. on Monday, December 5, 2016 (“Contest Period”). The contest will consist of a nomination submission period, nomination review period, finalist announcement, finalist video submission period, finalist online voting period and grand prize winner announcement as defined in the chart, below.
  2. HOW TO ENTER. To enter, Entrant must visit www.usafootball.com/footballtown and follow the links and instructions provided to register and nominate your Heads Up Football organization’s or high school team’s community as a Football Town nominee (an “Entry”), by the applicable Submission Period end date.

Entries, including subsequent submissions by Finalists, must be complete and submitted by the dates and times specified in the chart below to be considered. Entries generated by script, macro, or other automated means or practices, or by any means which subvert the entry process will be void.


If selected as a Finalist, the Nominee must submit additional materials as set forth below.



Sponsor will select 4 eligible Nominees (collectively, the “Finalists”, each a “Finalist”) that exemplify the pride, passion and tradition that earn it the designation of “Football Town”. Objective criteria that will be considered include: number of state or national championships won by local football programs, number of athletes from community that have gone on to play at an advanced level (ex. collegiate or professional football) and subjective criteria such as demonstrating rich annual football traditions (ex. Homecoming events, pep rallies, bonfires, etc.) will be considered. 


Using the information contained in the Entry, Sponsor will create an online voting profile for each Finalist that may be publicly viewed during the Online Voting Period. Votes may be cast by individuals of all ages worldwide. Voters will use the Sponsor-created voting platform to cast votes. Voters will be limited to one vote per device (i.e., computer, laptop, smartphone, tablet, etc.) per 24 hours.

Each Finalist has the option to submit a video by 11:59 p.m., E.S.T. on Friday, November 4, 2016 to include within their respective online voting profile that may be publicly viewed during the online voting period along with information from the Finalist’s Entry. The video should demonstrate how Finalist’s football program has pride, passion and tradition.


Any video submitted by the four Finalists becomes part of that organization’s Entry. Videos must: (1) be original; (2) less than 7 minutes long and edited to highlight the reasons you believe that organization’s community is a “Football Town” and exemplifies pride, passion and tradition; (3) be in keeping with Sponsor’s and Chase’s images; (4) not have been previously published, won another competition or contest or been used for advertising or promotional purposes; (5) be filmed in 2016; and (6) not infringe upon the copyrights, trademarks, rights of privacy, publicity or other intellectual property or other rights of any person or entity. 


Any video submissions that contain copyrighted music, video, photos, or other protected materials will not be posted for online voting. If a soundtrack is desired, use royalty-free/stock music or original compositions.


Late video submissions and submissions that do not comply with the requirements for a video submission that are subsequently corrected will be posted to Finalist’s voting profile by Sponsor as soon as practical but in any event no later than 2 business days following its submission in an approved format.    


Accompanying any video submitted must be a lawfully executed Authorizations to Use Name, Likeness, Image for each person identifiable in the video. Authorizations for minors must be signed by their parent or legal guardian. An Authorization form will be provided to all Nominees selected as Finalists for the contest.

By submitting an Entry (including the video), each Entrant agrees Sponsor shall own the Entry submitted (including all rights embodied therein) and that Sponsor and its designees may exploit, edit, modify, distribute, publicly display, and create derivative works from the Entry and all elements of such Entry, including, without limitation, the names and likenesses of any persons or locations embodied therein, in any and all media now known or hereafter devised, worldwide, in perpetuity without compensation, permission or notification to Entrant or any third party. If the Entry contains any material or elements that are not owned by the Entrant, and/or which are subject to the rights of third parties, the Entrant is responsible for obtaining, prior to submission of the Entry, any and all releases and consents necessary to permit the use and exhibition of the Entry by Sponsor in the manner set forth in these Official Rules, including, without limitation, name and likeness permissions from any person who appears in or is identifiable in the Entry. Sponsor reserves the right to request proof of these permissions in a form acceptable to Sponsor from any Entrant at any time. Failure to provide such proof, if requested, may render Entry null and void. By submitting an Entry, Entrant warrants and represents that he/she and any persons appearing or who are identifiable in the Entry have signed an Authorization to Use Name, Likeness, Image and that such Authorization has been submitted with the Video and consent to the submission and use of the Entry in the Contest and to its use as otherwise set forth herein. Sponsor reserves the right to disqualify any Entry that they determine, in their sole discretion, is offensive or inappropriate, not in keeping with Sponsor’s image or that is otherwise not in compliance with these Official Rules.

Entries may not contain, as determined by the Sponsor, in its sole discretion, any content that:

  • is sexually explicit or suggestive; unnecessarily violent or derogatory of any ethnic, racial, gender, religious, professional or age group; is profane or pornographic; or contains nudity;
  • promotes alcohol, illegal drugs, tobacco, firearms/weapons (or the use of any of the foregoing); promotes any activities that may appear unsafe or dangerous; promotes any particular political agenda or message;
  • is obscene or offensive; endorses any form of hate or hate group; appears to duplicate any other submitted Entries;
  • defames, misrepresents or contains disparaging remarks about other people or companies;
  • contains any infringing uses of trademarks, logos, or trade dress (such as distinctive packaging) owned by others, without permission. Entries may depict USA Football logos;
  • contains any personal identification, such as license plate numbers, full personal names, e-mail addresses or street addresses;
  • contains copyrighted materials owned by others (including photographs, sculptures, paintings, and other works of art or images published on or in websites, television, movies or other media), without permission;
  • contains materials embodying the names, likenesses, voices, or other indicia identifying any person, including, without limitation, celebrities and/or other public or private figures, living or dead, without permission;
  • contains look-alikes of celebrities or other public or private figures, living or dead;
  • communicates messages or images inconsistent with the positive images and/or good will to which we wish to associate; and/or
  • violates any law.

By submitting an Entry, Entrant acknowledges and agrees that Sponsor may obtain many Entries in connection with this Contest and/or other contests staged and/or promoted by the Sponsor, and that such Entries may be similar or identical in theme, idea, format or other respects to other Entries submitted in connection with this Contest and/or other contests sponsored by the Sponsor or submitted for other reasons or other materials developed by the Sponsor. Entrant waives any and all claims Entrant may have had, may have, and/or may have in the future, that any Entry and/or other works accepted, reviewed and/or used by the Entrant may be similar to his/her Entry, or that any compensation is due to Entrant in connection with such Entry or other works used by Sponsor.

By submitting an Entry, Entrant agrees that Released Parties (as defined above) are not responsible for any unauthorized use of Entries by third parties. By entering, each Entrant authorizes Sponsor to use his or her name, likeness, and image in their advertising and promotion. Released Parties do not guarantee the posting of any Entry. Entrants agree that they will not use the Entry for any other purpose, including, without limitation, posting the Entry to any online social networks, without the express consent of Sponsor in each instance. Sponsor does not guarantee the posting of any Entry and reserve the right, in their sole discretion, to remove any posted entry for any reason and at any time.

Proof of mailing or of certification (electronic or otherwise) does not constitute proof of delivery. Only one Entry per nominated organization will be eligible for prizes. By submitting an Entry, including the Finalist video, Entrant agrees to abide by these Official Rules. Persons submitting Entries under false pretenses will be disqualified. If for any reason the Internet or email portion of the contest is not capable of running as planned, including infection by a computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes which corrupt or affect the administration, security, fairness, integrity, or proper conduct of the Contest, Sponsor reserves the right, in its sole discretion, to disqualify any individual who tampers with the entry process, and/or to cancel, terminate, modify, or suspend the Contest. Sponsor has no obligation to notify an Entrant of his/her disqualification. Sponsor is not responsible for any lost, illegible, or misdirected Entries.  


  1. PRIVACY. Registration is required to enter the Contest. When you register, Sponsor collects personally identifying information about you, including your name, mailing address, and email address. The information that you provide will be added to Sponsor’s databases unless you explicitly opt-out of any non-contest related communications on the registration form. By entering and providing the required registration information without opting-out on the registration form, you acknowledge that Sponsor may send you information or special offers they believe may be of interest to you. Sponsor may also include your name and postal address in postal address lists that Sponsor sells or rent to third parties for marketing purposes. Your email address will not be sold or rented to third parties. For more information about how Sponsor uses the information you provide, see Sponsor’s privacy policy at http://www2.usafootball.com/pages/privacy_policy.

Sponsor has limited this Contest to adults age 18 and older. However, if an Entrant uploads a photo, video, or audio file containing images of children under the age of 13 or voice, or geological information sufficient to identify a location, city or town, full name, physical address, online contact information, or other information about a child under the age of 13, the Entrant is providing personal information that may be protected under the Children’s Online Privacy Protection Act (“COPPA”). Sponsor will use this information solely to administer this Contest as set forth in these Rules, including allowing the public to vote and to notify Contest Finalists and Grand Prize Winner. Sponsor will not sell or rent this information to third parties for marketing purposes.

Parents and legal guardians have the following rights with regard to their child’s personal information:

  • Sponsor does not seek Entries by persons under the age of 18 and therefore, will not require a child under the age of 13 to disclose more information than is reasonably necessary to participate in the Contest;
  • Sponsor will delete any contact information obtained pursuant to this Contest about a child under the age of 13 following the conclusion of the Contest Period.
  • Parents or guardians may review their Entry to review their child’s personal information and direct Sponsor to delete the information and refuse to allow further collection or use of the child’s information;
  • Parents or guardians may agree to the collection and use of their child’s information, but still not allow disclosure to third parties unless that is part of the service.
  • If a parent or legal guardian desires to exercise their rights, please contact USA Football’s General Counsel at 317.614.7750.

 Grand Prize Winner (1): The Finalist who has received the most votes at the end of the online voting period will be identified as the Grand Prize Winner. The Grand Prize Winner will be announced at 12:00 p.m. on Monday, December 5, 2016. The Grand Prize Winner will be awarded a $5,000 equipment grant from Sponsor. The $5,000 equipment grant is awarded as a credit with Sponsor’s partner Riddell and is redeemable for helmets, shoulder pads, uniforms or other various football related items (organizations will not receive monetary grants).


  1. NOTIFICATION OF WINNERS. Sponsor will contact the Finalists by the phone, mail, and/or e-mail address that was provided to Sponsor by 5:00 p.m. on Friday, October 14, 2016. The Finalists must acknowledge the communication and return an Affidavit of Eligibility/Ownership and a Liability/Publicity Release within 72 hours.

Sponsor will contact the Grand Prize Winner by phone, mail, and/or e-mail by 5:00 p.m. on Monday, December 5, 2016. The Grand Prize Winner must acknowledge the communication within 72 hours. Sponsor will then provide the Grand Prize Winner with an Affidavit of Eligibility/Ownership, an IRS W9 form, and a Liability/Publicity Release. The Grand Prize Winner will accept the Grand Prize by signing and returning the Affidavit no later than 72 hours after receipt. The Grand Prize will not be awarded until such documentation is received by Sponsor. The Grand Prize Winner is responsible for any taxes associated with the award of the Grand Prize. In the event that a potential Grand Prize Winner does not sign and return the Affidavit within 72 hours after receipt, Sponsor will offer the Grand Prize to the Finalist receiving the next highest number of votes, and he/she will be contacted as set forth above. The Grand Prize for the Grand Prize Winner will not be awarded until the completion of the Contest Period. Unclaimed prizes may not be awarded.

  1. PRIZE CONDITIONS: Expenses not stated are solely the responsibility of the Grand Prize Winner (collectively, “Winning Participant”). No prize may be transferred or substituted by a participant. Sponsor reserves the right to change any actual prize at any time during the Contest. In this event, every effort will be made to provide a prize of like estimated value. Sponsor does not warrant or make any representations in connection with the prize, express or implied. Sponsor will determine all elements of any prize in its sole discretion. Prizes may not be assigned, transferred, or changed, except at the sole discretion of Sponsor. The awarding of any prize is contingent upon full compliance with these Official Rules. All prize details is at Sponsor’s sole discretion.
  2. Everything for this contest is provided as is, without warranty of any kind, express or implied. By entering, participants consent to the exclusive venue and jurisdiction of the state and federal courts of Indianapolis, Indiana, in all disputes arising out of or related to these rules or these contests.
